Laparoscopic Splenectomy for Hairy Cell Leukemia in Pregnancy
نویسندگان
چکیده
Objective. We present a successful case of laparoscopic splenectomy for a massively enlarged spleen at 25 weeks of gestation for hairy cell leukemia in pregnancy in a woman with initial hemoglobin of 4.3 gm/dl and platelet count of 18,000/mm(3). Study Design. Case report. Results. This report provides an approach to management that may be applicable in those cases where thrombocytopenia or other clinical imperatives preclude delaying treatment till after pregnancy. Conclusion. Hairy cell leukemia is a clonal B-Cell malignancy, for which there is very limited experience worldwide for its management when it occurs during pregnancy. Laparoscopic splenectomy should be considered as a therapeutic option, even with a significantly enlarged spleen, in order to avoid the risks of fetal exposure to chemotherapeutic agents. Unique considerations relating to pregnancy are highlighted.

A Case of Hairy Cell Leukemia Associated with Hepatitis B Surface (HBs) Antigenemia and Concurrent Hepatitis C Virus (HCV) Seropositivity
Literature review shows that hepatitis C infection may increase the risk of non-hodgkin lymphoma. Reactivation of hepatitis B infection has been reported in patients with hairy cell leukemia after chemotherapy and/or splenectomy. We present hepatitis B surface (HBs) antigenemia and concurrent hepatitis C virus (HCV) seropositivity in a case of hairy cell leukemia. Key Words: Hairy cell leukemi...
متن کاملSevere platelet dysfunction in hairy cell leukemia with improvement after splenectomy.
A patient with hairy cell leukemia developed purpura not attributable to thrombocytopenia. We found markedly reduced platelet aggregation responses and malondialdehyde production, decreased serotonin uptake, and depleted dense granule contents. Ultrastructural studies showed that most platelets had few or nor granules. All of the clinical and laboratory studies of platelet function and morpholo...
متن کاملDistinct subtype within the spectrum of hairy cell leukemia.
Most cases of hairy cell leukemia represent malignancies of B cells. However, recent findings suggest that there is a spectrum of functional capacities within the entity hairy cell leukemia. Two patients with hairy cell leukemia, whose malignant cells in the peripheral blood showed both T- and B-cell features, are reported. The malignant cells of the spleens showed only B-cell characteristics. ...
متن کاملResponse to splenectomy in 65 patients with hairy cell leukemia: an evaluation of spleen weight and bone marrow involvement.
Sixty-five patients with hairy cell leukemia underwent splenectomy; 27 had a complete remission as defined by a return in WBC, RBC, and platelet counts to a defined level, and 38 had a partial remission with a return of only one or two of these parameters to the defined level. The 5-yr actuarial survival for all patients is 68%; for CR patients it is 76%, and for PR patients 62%. The response t...
متن کاملHigh-dose methotrexate with leucovorin rescue: effectiveness in relapsed hairy cell leukemia.
Six patients with relapsed hairy cell leukemia after splenectomy were treated with high-dose methotrexate and leucovorin rescue. Five patients had objective responses as determined by improved blood counts. In two of them, the response has continued for more than 14 and 44 months, respectively. Intensive treatment with high-dose methotrexate and leucovorin rescue was well tolerated and no signi...
